- Page 2 – Festival (Write in “BandFair”)
- Page 6 – Preschool, Private Elementary, Private Middle School (choose North Branch School from the list)
- Page 6 – Preschool teacher, Elementary teacher, Middle School teacher (Write in your choice)
- You don’t need to vote for all of the many categories in the survey, but you do need to go to the end and hit submit.
